New Malignancy Referral– Estimated time for appointment 24-72 hours
- Insurance information (to assess for referral requirements and to evaluate coverage for
anticipated treatment) - Pertinent imaging
- Pertinent labs
- Op Note
- Pathology
- Office note(s)
Hematology Referral (Anemia, Polycythemia, Leukocytosis, etc.)
Allow up to 7-10 days to review and schedule
- Insurance information (to assess for referral requirements and to evaluate coverage for anticipated treatment)
- Pertinent labs. Please include serial labs (min. 3 months if available) so we can follow trend
